me late que tienes problemas con el codigo .css.
Deberçias incluirlo igual aquí para poder revisar ya que acá no encuentro ningun error aparente.

hermano pille la solucion.
Acá te dejo el formulario como debe ser, las lineas colspan 2 molestaban y generaban una doble columna. lo que no te permitia que el otro quede igual:
